Guest Waivers

The MSC Liability Waiver Policy
Everyone who participates in, or attends, any Club sponsored event must have previously signed a liability Waiver. This protects the Club officers, Club members, and any employees or agents from liability associated with club activities.
The Club policy applies to all Members in good standing, their family members and dependents.
It also applies to all Guests, visitors, friends, etc.

Follow this link to view a list of guests (et all) who have already signed a Forever-Waiver. These signed Waivers do not expire. No further action is required for the names on this list. The waiver covers all future events.

If a Wavier is required for a guest follow this link for a blank form.
Please read, sign & date the Waiver form. Give the signed Waiver to the event host, any officer or a member of the club. You can also scan the waiver and email it to:

Your name will be added to the list above and the Waiver will be stored with the club records. It will cover you for future events.
